GVTA

You may fill in the “Coverage Beyond Regular Duty Report” to report times when you were asked to provide coverage beyond your regular job duties, usually in the event of an unfilled substitute position. The form will be confidential and received only by the president of GVTA.

Coverage Beyond Regular Duty Report

The “GVTA Expense Reimbursement” form should be printed on blue paper. Please tape your receipt to the back (top and bottom of receipt), or use a staple if the receipt is a full page. Forms can be sent to your committee chair, or to Anny Froese at PDS.

Workplace Safety & Health

The division has two Violent Incident Report Forms – One located in Policy, and one located in the Workplace Safety & Health 11 Elements document. It is not important which form you use; what is important is that you report workplace violence and get help for both you and the perpetrator.
